What percentage of young people with EHC Plans who turned 18 during the 2023/24 academic year had been assessed under the Care Act by their 18th Birthday?

I am conducting research as part of a lecture I am holding for Social Work Students so I am requesting information you hold on the percentage of young people whom turned 18 during the 2023/24 academic year (1st September 2023 - 31st August 2024) whom held Education and Health Care Plans at the time and who were assessed under the Care Act by their 18th birthday to determine their eligibility in preparation for a transition into Adult Social Care Services.I wish to compile this information as part of a study into the reasons why many young people with disabilities may or may not be assessed by their 18th birthday so if you have any explanations for the percentage you produce then this would be greatly appreciated.ThanksTom
